The Patient Champion is a point of contact at the practice for patients who need extra help and support. In particular patients who are identified as carers are signposted to our Patient Champion who provides them with a Carers Pack. Our carers pack hold information on local services which can offer extra support. Our patient champion also provides Mental Health Packs for any patients who are suffering with their mental health. These packs also provide information on local services where patients can seek extra support and help in their time of need. Our patient champion will also signpost patients to services which includes the practice social prescriber.
The Patient champion also acts as our Cancer Champion and focuses on increasing the uptake of breast cancer screening, bowel cancer screening and cervical cancer screening at our practice by contacting patients who have not responded to invites for screening and provide encouragement and extra support to patients to engage with these important screening programmes.
